Education Freedom Account Application

Application Overview

Education Freedom Accounts (EFA) are grants from the State of New Hampshire, administered by CSF, made available to families to spend on their children's education, including tuition at the school of their choice, tutoring, classes, online learning programs, educational supplies, technology, and other educational expenses.

Please complete this application in full and follow all of the instructions. You will have an opportunity to upload your supporting documents while completing this application. All documents should be in PDF, PNG, JPEG, or JPG file formats.

Helpful Tips

  • The Parent and student names must match the documents that are submitted to be valid.
  • When completing this application form, do not type in ALL CAPS in the text fields.
  • Do not include commas in the income fields.
  • Convert your documents into PDF, PNG, JPEG, or JPG file formats prior to starting your application.

Required Documents

During the application process, you will need to provide at least one of the following documents for each section:

If you wish, you can include your income and household size information to potentially qualify for additional grant funding, and/or for your student to qualify in a priority group, enabling them to enroll even if we hit the cap of students for the program.

Proof of New Hampshire Residency

Provide either one Primary document OR two Secondary documents:
Primary (submit one)
  • Utility bill (water, electric, gas, cable, landline phone, or internet) issued within the past 60 days
  • Address Confidentiality Program (ACP) identification card
  • Government Documents - Medicaid or SNAP showing your name and address
  • Tax Return
  • Paystub (issued within the past 60 days)
Secondary (submit two)
  • Valid New Hampshire Driver’s License
  • State-issued Identification Card
  • W2
  • Mortgage statement
  • Rental or Lease Agreement
  • Property Tax Bill or other Town/City documentation
  • Vehicle Registration or Insurance Policy
  • Legal Guardianship, Foster Care, or DCYF documentation
  • Bank Statement
  • Permanent Change of Station (PCS) Military Orders
  • Signed and notarized Affidavit of Shared Residence from the property owner (not a roommate)

Income Verification (if applying for priority status)

  • Federal 1040 tax return from the prior year.
  • SNAP/TANF Budget Letter showing the Adjusted Benefit Amount
  • SSA-1099/SSI letters for family members receiving benefits

Household Documents (if applying for priority status)

  • Document listing names of everyone in the household
  • Federal 1040 with all dependents listed
  • SNAP/TANF documentation with all household members listed

Student Documentation (Required)

  • Birth certificate for each student
  • Student ID for each student
  • Federal 1040 tax return with dependents listed
  • SNAP/TANF with each student listed
  • Legal guardianship documentation (if you are a legal guardian)

Student Documentation (Optional)

  • Student's IEP, special education evaluation by public school district, or licensed medical professional certification of qualifying disability (if requesting additional grant funding, and/or for your student to qualify in a priority group)

Please submit only one (1) application per family. Submitting multiple applications may delay processing times.

CSF maintains the confidentiality of Personally Identifiable Information (PII) in accordance with state and federal law and requires that third-party vendors maintain the confidentiality of PII in accordance with state and federal law.
